Service Boundaries


Central West Case Management Unit offers programs to enable older adults to live as independently as possible and assists family and friends in caregiving responsibilities.

Our professionally-trained staff of case managers perform in-home and hospital-based assessments to evaluate an older person's needs and resources, secure and link them with needed services, and provide ongoing monitoring of the individual plan of care.


  • Provide quality case management services to frail, older adults living in Chicago's west side neighborhoods
  • Educate the University and larger community through presentations and training
  • Provide opportunities for students and faculty to gain insight into gerontological practice
  • Support innovative models of service, research and evaluation
  • Provide MSW students on-site experience working with older adults

Plan of Care

Through the Illinois Department on Aging Choices for Care program, case managers visit all west side medical centers to assess the older adult prior to discharge. An older adult is able to choose community-based services, if their medical and physical abilities allow, as alternatives to long-term, institutional care.

The older adult, significant others, and the case manager develop an individual plan of care that includes all identified areas which assistance may be needed, including help with activities of daily living, personal hygiene, and household chores.

Service Boundaries

The Central West Case Management Unit serves older adults residing south of North Avenue, north of the Sanitary Canal, and east of Austin Avenue to Lake Michigan.
